GloryBee in Eugene, Oregon, has each employee have a dialogue with his/her immediate supervisor annually.  The purpose of the dialogue is to provide feedback to management about why people work at GloryBee, and what management can do to increase their attachment to the company.  They use a computer-fillable Excel form.  The content is reproduced here:  GloryBee Just Ask 2-page 141103
